    The race between education and catastrophe: creating climate-sensitive cities

    ‘History is a race between education and catastrophe’ (Gurría, 2013). This is the essence of the climate-change dilemma in human settlements today. How can cities act effectively to live with climate change? After a century, the world is bracing for a perfect storm with burgeoning populations drifting to the cities, resulting in anthropogenic greenhouse gases multiplying exponentially. Rapid urban development breaks down natural systems that sequester greenhouse gases, making cities unhealthy, unbalanced, and undesirable (Blakely & Carbonell, 2012). It is reported that ‘more people are killed from poor urban design and climate-change than terrorism’ (Birkeland, 2008), which is an immense ethical problem. My focus is on creating resilient Climate-Sensitive-Cities®. This approach to tackling the urgency of climate change in cities is five-fold: 1. It audits vulnerability and coping capacity. 2. It addresses audit gaps by investing in accelerated learning for professions and communities to build capacity for resilience. 3. It appreciates that every individual can make a unique contribution to interdisciplinary capacity-building for addressing climate education through their own organisations and regions. 4. It realises that accelerated learning for the long-term investment in individuals within organisations includes collaborative coaching and partnering. 5. It recognises that optimism for a preferred future can be achieved in a world full of perverse incentives. Although there are many perspectives and prescribed actions from each discipline, my approach is founded on meta-scanning, with principle-based options that emerge from broad lessons from international and local successes. The crucial part of my work is to convert research into desirable actions in a way that demonstrates learning for better climate governance. The results of my endeavours include influence of policy and practices in fourteen countries and through professional bodies across disciplines. My contribution to transformational guidelines for international climate action transparency is recognised widely. This thesis comprises an exploration of philosophies, revisiting values, seeking answers to four research questions, a new lens with three perspectives, and project design to ensure higher fidelity with my statement of intent. My Doctorate establishes a framework that enables individuals to lead the way in climate-change practices. I intend to be a living example of such frameworks. The thesis concludes with new definitions for Climate-Sensitive-Cities® and Accelerated Learning. It also overlays triple loop policy development with the Climate Policy in Practice Cycle® as a means for funding and evaluating action. Finally, a manual for Master Classes delivered across fourteen countries works with a transformational change trajectory that articulates the journey from passive bystanding, to advocacy, to tipping points, to coping with success. Independent evaluations accelerate the uptake of these skills in governments, communities, professions, and most importantly, individuals. We need to learn, question old thinking, and relearn in order to adapt and live with the many facets of climate-change. Based on this rationale, I have structured my Doctorate to advance a deeper understanding of the technical, intellectual, and interpersonal skills required of an effective Sustainability Commissioner. Alvin Toffler stated ‘the illiterate of the 21st Century will not be those who cannot read and write, but those who cannot learn, unlearn, and relearn’ (ISLS, 2013).     Raloxifene and Selective Cell Cycle Specific Agents: A Case for the Inclusion of Raloxifene in Current Breast Cancer Treatment Therapies

    BACKGROUND: Breast cancer patients are at increased risk of osteoporosis. Contributing factors include age and/or chemotherapy. The selective estrogen modulator, raloxifene (RAL), effective in the prevention of breast cancer and approved for the treatment and prevention of osteoporosis, may prove beneficial in current breast cancer treatment modules. The purpose of this study was to evaluate RAL in combination with 5-fluorouracil (5-FU) and trimetrexate (TMX) to determine the most effective sequence in which to administer these cell cycle specific agents while taking into consideration the cellular mechanism of action. The goal was to maintain cytotoxicity to breast cancer cells and capitalize on the selective estrogen receptor modulatory effects of RAL. MATERIALS AND METHODS: MCF-7 cells were exposed to (i) TMX, 5-FU or RAL alone, or (ii) RAL 24 h prior to 5-FU followed 2 h later by TMX, or (iii) 5-FU 2 h prior to TMX followed 24 h later by RAL. The cell viability was determined using the Quick Cell Proliferation Assay. RESULTS: The growth rate of MCF- 7 cells exposed to early RAL was 68.25 +/- 4.11% that of the control, however, late RAL exposure produced a growth of 34.75 +/- 4.79% that of the control. Late RAL maintained the cytotoxicity of the regimen. The findings were further supported by cell flow cytometry and Western blot analysis data. CONCLUSION: RAL given prior to 5-FU/TMX significantly compromised cytotoxicity to breast cancer cells

    Trends in Gender Authorship and Collaborations: A 30-Year Comparative Bibliometric Analysis of Manuscripts from The Journal of Bone and Joint Surgery and The Bone and Joint Journal

    Publishing original peer-reviewed research is essential for advancement through all career stages. Fewer women than men hold senior-level positions in academic medicine and, therefore, examining publication trends relative to gender is important. The goal of this study was to examine and compare publication trends in The Journal of Bone and Joint Surgery (JBJS) and The Bone and Joint Journal (BJJ) with a particular emphasis on trends regarding author gender. Data was collected and analyzed for manuscripts published in JBJS and BJJ over the past 30 years. For manuscripts published in 1986, 1996, 2006, and 2016, we recorded the numbers of authors, manuscript pages, references, collaborating institutions, the position in the byline of the corresponding author, the country of the corresponding author, and the names of the first and corresponding author. We also calculated the normalized number of citations and corresponding author position. The number of authors, institutions, and countries collaborating on manuscripts published in both JBJS and BJJ increased over time. JBJS published more manuscripts from North America and BJJ published more manuscripts from Europe. In both journals, the percentage of women as first and/or corresponding author increased over time. Trends over the past 30 years have shown increased collaborations with greater citations in manuscripts published in JBJS and BJJ. In the same time period, both journals demonstrated a rise in the percentage of manuscripts with women first and/or corresponding authors, suggesting a decrease in the gender gap

    Clinical practice: Noninvasive respiratory support in newborns

    The most important goal of introducing noninvasive ventilation (NIV) has been to decrease the need for intubation and, therefore, mechanical ventilation in newborns. As a result, this technique may reduce the incidence of bronchopulmonary dysplasia (BPD). In addition to nasal CPAP, improvements in sensors and flow delivery systems have resulted in the introduction of a variety of other types of NIV. For the optimal application of these novelties, a thorough physiological knowledge of mechanics of the respiratory system is necessary. In this overview, the modern insights of noninvasive respiratory therapy in newborns are discussed. These aspects include respiratory support in the delivery room; conventional and modern nCPAP; humidified, heated, and high-flow nasal cannula ventilation; and nasal intermittent positive pressure ventilation. Finally, an algorithm is presented describing common practice in taking care of respiratory distress in prematurely born infants

    Small molecules, big targets: drug discovery faces the protein-protein interaction challenge.

    Protein-protein interactions (PPIs) are of pivotal importance in the regulation of biological systems and are consequently implicated in the development of disease states. Recent work has begun to show that, with the right tools, certain classes of PPI can yield to the efforts of medicinal chemists to develop inhibitors, and the first PPI inhibitors have reached clinical development. In this Review, we describe the research leading to these breakthroughs and highlight the existence of groups of structurally related PPIs within the PPI target class.     CUORE and CUORE-0 experiments

    Neutrino oscillation experiments proved that neutrinos have mass and this enhanced the interest in neutrinoless double-beta decay (0vßß). The observation of this very rare hypothetical decay would prove the leptonic number violation and would give us indications about neutrinos mass hierarchy and absolute mass scale. CUORE (Cryogenic Underground Observatory for Rare Events) is an array of 988 crystals of TeO2, for a total sensitive mass of 741 kg. Its goal is the observation of 0vßß of 130Te. The crystals, placed into the a dilution cryostat, are operated as bolometers at a temperature close to 10 mK. CUORE commissioning phase has been concluded recently in Gran Sasso National Laboratory, Italy, and data taking is expected to start in spring 2017. If target background rate is reached (0.01counts/day/keV/kg), the sensibility of CUORE will be, in five years of data taking, T1/21026years (1? CL). In order to test the quality of materials and optimize the construction procedures, the collaboration realized CUORE-0, that took data from spring of 2013 to summer 2015. Here, after a brief description of CUORE, I report its commissioning status and CUORE-0 results

    Lowering the CUORE energy threshold

    The Cryogenic Underground Observatory for Rare Events (CUORE) is a ton-scale double beta decay experiment based on TeO2 cryogenic bolometers and is currently in the last construction stage at the Gran Sasso National Laboratory (LNGS). Its primary goal is to observe neutrino-less double beta decay of 130Te, however thanks to the ultra-low background and large projected exposure it could also be suitable for other rare event searches, as the detection of solar axions, neutrinos from type II supernovae or direct detection of dark matter. The sensitivity for these searches will depend on the performance achieved at the low energy threshold. For this reason a trigger algorithm based on continuous data filtering has been developed which will allow lowering the threshold down to the few keV region. The new trigger has been tested in CUORE-0, a single-tower CUORE prototype consisting of 52 TeO2 bolometers and recently concluded, and here we present the results in terms of trigger efficiency, data selection and low-energy calibration

    Status and prospects for CUORE

    CUORE is a cryogenic detector consisting of 988 TeO2 crystals, 750 g each, and will be operated at a temperature of ~10 mK, to search for neutrinoless double beta decay (0¿ßß) of 130Te. The detector, in the final stages of construction at the Laboratori Nazionali del Gran Sasso (Italy), will start its operations in 2016. CUORE-0, its pilot experiment, has proven the feasibility of CUORE, demonstrating that the target background of 0.01 counts/keV/kg/y and the energy resolution of 5 keV are within reach. CUORE-0 also made the most precise measurement of the 2¿ßß decay. The expected sensitivity of CUORE to the 0¿ßß 130Te half-life is 9 •1025y, for 5 years of data taking. Here, we report the most recent results of CUORE-0, their implications for CUORE, and the current status of the CUORE experiment
